+ Complete change in procedures: When the entire process of cultivation, harvesting, extraction, or preliminary processing is replaced.

As stipulated in Article 9 of Circular No. 19/2019/TT-BYT, the dossier used for periodic assessment of continued compliance with good agricultural and collection practices (GACP) must include:

+ A written request for assessment.

+ A certified copy of the Decision on Establishment, Business Registration Certificate, or Investment License (applicable to cases in Point a, Clause 1, Article 8 of the Circular).

+ The organizational structure and personnel list of the facility. In case the facility consists of many households, it is necessary to send a list of names, addresses, locations, cultivation and harvesting areas and purchase contracts between the facility and the households/representatives of the households.

+ A lot map showing cultivation or harvesting zones, with a detailed description of these areas including processing and storage facilities.

+ Plan, content and results of GACP training for the facility's staff in the year of application.

+ The process of cultivating, harvesting and processing medicinal plants developed by the research facility or applying the results of scientific research or published and issued documents.

+ Results of the facility’s self-assessment based on the Evaluation Criteria Table (Appendix II of Circular No. 19/2019/TT-BYT).

+ A certified copy of the list of forest products confirmed by the Forestry Department.

III. Questions on assessment of compliance with good agricultural and collection practices (GACP) standards 

1. What information is required in the assessment dossier of compliance with good agricultural and collection practices (GACP) standards ?

Pursuant to Clause 1, Article 18 of Circular No. 19/2019/TT-BYT, assessment of compliance with good agricultural and collection practices (GACP) shall include the following entities: 

+ The Head and Secretary from the receiving authority;

+ At least one representative from the Institute of Medicinal Materials;

+ One representative from the provincial Department of Health where the site is located;

+ At least one additional member from the receiving authority;

+ Other members are appointed based on the technical needs of the assessment.
